Review Form Problem

Dear Sir/Madam,

I couldn't add any items to a "Review Form". When I click to "FORM ITEMS" link in order to add new form items to the form, a fully blank screen shown.
Therefore, I could not add any items to the Review Form. How can I solve this problem?

See this FAQ entry; also, please let us know what version of OJS you're using.
