The plugin writes all changes to the same css file. 

Jackie says it best: 
"We have a single installation of OJS with multiple journals. Two of the journals would like to use the Custom Theme plugin to set their background colours, however I have discovered that when the second journal manager saves the colours selected in their plugin, this is reflected in the first journal even though the colour selection appears to remain in their journal manager’s setup."

Here's line 113 of CustomThemeSettingsForm.inc.php: 
   FileManager::writeFile(dirname(__FILE__) . '/custom.css', $css);
