I have developed a review form in OJS 18.104.22.168. In all cases, data are present in the completed form, but in 2 our of 3 cases the reviewer decision is not recorded (recommendation still says "none" and the acknowledgement email icon is grayed out. In the 3rd case with the same form everything worked as expected. That reviewer found no problems in working with it. How can I begin to troubleshoot this??

Have you double-checked that the reviewer actually recorded the review after completing the form? Apparently reviewers miss this step sometimes.
