What to do if you have a technical problem with OCS:
1. Search the forum

I am trying to perform a Full Package upgrade from OCS 18.104.22.168 to OCS 22.214.171.124 and receive the following error when trying to perform the DB upgrade from either the web interface or CLI:
ERROR: Upgrade failed: DB: Duplicate entry 'REVIEW_ACK-0-1' for key 2
Can this problem be related to another issue experienced on this OCS site, logged here: viewtopic.php?f=3&t=5980

This is likely caused by an upgrade on an already failed upgrade (thus on a fractured database). Please try restoring from backup to a stable state, then trying to upgrade again.
