I would like to have the ability add nested sections (sub sections).

I did some quick research and the only reference I could see to a request like this is here:

California Digital Library has also received this request from journal users.

We would also very much like to have this feature. For the purpose of Polish-origin Journal of Rare Cardiovascular Diseases it is crucial to support sections with "general disease classification". For example:

Original Article / rare diseases of the heart
Original Article / rare diseases of systemic circulation
etc.
Case Report / rare diseases of the heart
Case Report / rare diseases of systemic circulation
etc.
