Plugin to extend user registration form with additional custom fields

Allow JMs to create additional fields to include as part of the user registration form. This would enable a journal to capture additional information about its readership, e.g. - "How did you hear about the journal?" - "Which topics would you like to see better covered?" - etc. JMs should be able to create new fields of various types (e.g. text field, radio, checkbox, etc.) Provide a summary view for fields. Should be able to re-use much of the review form schema and code.
An alternative approach would be via a basic survey plugin. JMs would be able to create short surveys, which could be displayed on a separate page and optionally linked from the user registration page. Links to active surveys could also be displayed to readers upon login on the main user home page. Review form re-use would equally apply here as well.
