Add notification for submission file uploads

There should be a notification available from the Notification Settings options for any new submission file uploads/modifications. This should include at the very least a notification to the editor when an author has uploaded a revised submission file for review. This is primarily so that Editors have a better idea on when and whether files have been updated, without necessarily relying on users to notify them manually.
Same for OCS (http://pkp.sfu.ca/support/forum/viewtopic.php?f=3&t=7862&p=30483#p30483), and presumably OMP.
Several CDL journals have requested this as well. More broadly, it seems that there are several places throughout OJS where the user is expected to notify someone that a task is done, but that notification workflow is very easy to overlook or ignore. It'd be great to do an analysis of all the places this is so, and make automatic notifications an option. I'd also like to suggest that this be a configurable option.
