Currently the homepage <title /> is fixed at common.openMonographPress|translate -- coming from PKPTemplateManager from $application->getNameKey(). Need to allow for the homepage title to be set (e.g. to the press name, if available).

I've changed this so the press index page will now use the 'name' setting for the Press, instead of "Open Monograph Press".  Other pages still use their breadcrumb titles.
