User interests controlled vocab entries check assoc_id without specifying assoc_type. This may lead to collisions between controlled vocab entries used for different purposes and also prevents the database from using the related indexes.

Corrected for all apps. To fix this for OJS 2.3.3-2, for example, both attachment #3297 [details] (in lib/pkp) and attachment #3296 [details] (in the install directory) need to be applied.
